Aftercare Services
At Adefey Care, we are proud to support individuals eligible for Section 117 Aftercare under the Mental Health Act 1983. Our tailored service provides high-quality, person-centered care for those discharged from hospital following detention under sections 3, 37, 45A, 47, or 48, ensuring their needs are met with compassion, dignity, and respect.
Section 117 Aftercare is a legal entitlement, and we work in close partnership with local authorities, NHS trusts, and multidisciplinary teams to ensure seamless transitions from hospital to community-based support.
What We Offer:
– Specialist Supported Living or Community-Based Placements tailored to mental health recovery.
– Access to a dedicated support team, including key workers and managers experienced in mental health and complex needs.
– Promotion of independence, stability, and meaningful community integration.
– Holistic support covering mental and physical health, housing, finances, education, employment, and social connection.
– Ongoing risk management, safeguarding, and recovery planning in line with the Care Programme Approach (CPA).
– Clear communication and coordination with care coordinators, AMHPs, families, and advocacy services.
Our goal is to empower each individual to reclaim their independence and live a fulfilled life beyond hospital care. Whether short-term recovery or long-term stability is the focus, Adefey Care delivers trusted, regulated, and consistent aftercare with a strong focus on outcomes and quality of life.
We currently offer Section 117 Aftercare placements across Yorkshire, Humber, Lincolnshire, and the North West.
